Requesting Your Health Care Records

You have the right to request a copy of your health care records by submitting a written request.

To request records from Atascosa Counseling and Consultation Services:

• Submit a written request to our office
  
  

• Requests may be completed using our records request form or by contacting our administrative staff
  
  

• Requests are processed within the timeframes allowed by Texas law
